Enhanced Care Coordinator

Community Care Durham seeks an energetic team player with a strong background in community social work to build collaborative working relationships with representatives of hospitals and community organizations; work as a member of multidisciplinary professional teams at locations within Durham Region, and work cooperatively with Community Care Durham staff to facilitate scheduling and support of staff to provide client care at home.
The one year contract position is full time, 35 hours per week; continuing employment is contingent on funding as well as demonstrated program success in achieving objectives.
Reporting to the Specialized Geriatrics Manager, the Enhanced Care Coordinator evaluates client eligibility; provides support and guidance to staff to ensure the program mandate is met; evaluates outcomes; and participates as an effective member of the Community Care Durham and partnership teams.
Qualifications required:
· Undergraduate degree in health/human services/social work or a related field with three years experience in a related role; equivalent combination of education and recent related experience considered. Registration with the Ontario College for the profession required.
· Sensitivity to working with aging adults and persons with disabilities
· Experience with inter-organizational cooperation and partnerships; experience working within an inter-professional team environment
· Comprehensive experience in assessment, case management, problem solving and case documentation
· Well developed interpersonal, communication and presentation skills; tact and diplomacy; facilitation and negotiating skills; initiative and flexibility
· Ability to organize work and set priorities; ability to meet deadlines
· Intermediate level Microsoft Office skills
· Knowledge of community support resources within Durham Region a definite asset
· Licensed vehicle and the ability to travel within Durham Region
· Satisfactory criminal reference check including vulnerable sector screening
Compensation: $ 24.97 – 29.85 per hour; 4 % vacation pay
