Assessment & Review Coordinators

Family Service Toronto is the designated agency for the Passport Program in Toronto. “Passport to Community Living” is a Provincial Government-funded initiative that provides opportunities for adults who have a developmental disability to receive support to participate in their communities. We are looking for two experienced, resourceful and creative persons to join our team for a five-month period.
RESPONSIBILITIES:
Working individually and within a team that includes Passport colleagues and the Manager, you will:
  • Hold Passport Application Review meetings with the individual and their family to discuss the application and to provide information that will assist the applicants to make informed choices
  • Apply extensive knowledge of Toronto Developmental Services and an understanding of community inclusion to assist applicants to access the supports and services they require to achieve their goals
  • Apply the program assessment tools and guidelines
  • Receive updates from applicants and conduct reviews as required
  • Collaborate with colleagues in planning, training and review meetings
  • Prepare file records, statistical reports and other administrative tasks as required in a time-sensitive manner
QUALIFICATIONS:
  • A post-secondary degree in social work or an equivalent combination of education and experience over a minimum three-year period (foreign credentials and relevant experience will be considered)
  • Minimum three years of experience in the social services field
  • Demonstrated experience in applying assessment tools with sound professional judgment
  • Sound understanding of issues faced by people with developmental disabilities
  • Knowledge of Person Directed Planning
  • Extensive knowledge of community-based resources and the developmental services system
  • Strong interpersonal and communication skills (verbal, written and listening)
  • Ability to work independently as well as in a team dynamic
  • Good organizational, time management and computer skills (familiarity with database applications)
  • Able to work with numbers, budgets, statistical reporting, and other administrative tasks in a fast-paced environment with overlapping deadlines
  • Fluency in one of the following languages: Farsi, Cantonese, Tamil or Spanish
  • A valid Ontario driver’s license and access to a vehicle is an asset
LOCATION: 700 Lawrence Avenue West
TRAVEL: Some travel within Toronto may be required
SALARY RANGE: Salary Grade 7 Bargaining Unit (pro-rated to time worked)
$46,991 - $57,856
